Brazilian Tourist Visa Requirements
for Canadian Citizens:
1. Passport Requirements: Original, signed Canadian passport valid for 6 months beyond the day you land in Brazil, and with at least one blank visa page available for the Brazilian visa stamp. Amendment pages in the back of the passport are not suitable for Brazil visas.
2. Brazilian Visa Application Form:
One original Brazil visa application per applicant properly completed on-line, printed and signed, plus one copy. Only on-line Brazilian visa applications are accepted. Signature must be original on printed Brazil visa application receipt.
Note: Improperly completed Brazil visa application forms may delay the processing of your visa. To fill in the on-line form select "Visa Request" and follow the steps. Every question must be answered. The signature must be inside the signature box on the receipt page. The form will expire within 30 days; the visa application must be submitted within that time or a new on-line Brazil visa application form must be completed and printed. The Brazilian Consulate will only accept on-line visa application forms. Residents of CT and RI must have signature on application receipt notarized.
3. Photo Requirements: One recent 2 x 2 passport type photograph, in color, front view and with a plain/white background. OR - you can upload a passport photo to the Global Service Order Form when you place your order, instead of providing physical passport photos.
4. Proof Of Departure: Computer generated flight itinerary from the airline or travel agency or copy of airline tickets showing round trip travel to Brazil and onward flight.
5. Financial Requirements: Bank statement or copy of last income tax (Revenue Canada - Notice of Assessment).
6. Global Service Order Form: This is Travisa's service order form, where you will provide your contact and shipping information, choose the visa processing needed and make your payment. You must print the completed form and send it to Travisa with all other supporting documents.
(Travisa is one option for obtaining a Visa. You can choose other options.)
7. Personal Letter: A personal letter from the applicant addressed to the Embassy of Brazil, explaining the purpose of the trip, dates of travel, and cities to be visited. Stating "tourism" only is fine, you need not mention going to John of God or a healing center.
For travelers under 18 (in addition):
1. Affidavit (for travelers under 18 years old). Notarized affidavit of financial support stating that the parents are responsible financially for the minor's trip.
2. Long Birth Certificate (for travelers under 18 years old). Copy of the child's long birth certificate with the parent's name on it.
3. Authorization form (for travelers under 18 years old). Form authorizing that both parents are allowing the minor to apply for a Brazilian Visa.
4. Polio vaccination (children between the ages of 3 months and 6 years). Copy of the certificate against Polio
